Sample Submittal Form

In order to properly evaluate recyclable materials, please complete the sample submittal form for each material you wish to submit for recycling review. Return the completed form to Ardleigh Minerals, along with any available analytic data, an MSDS, and a one-pound representative sample of each material.

When submitting samples of plastic, please indicate the type (T5, T2, etc.).

When submitting samples of separately collected steel shot and shot dust, please submit separate, individual samples; send a two-pound sample of each.
